You are correct about the wimpiness of the aural alert with the engine running. It's really only useful in the self-test.
I don't routinely monitor 121.5. I flip-flop the two comm radios all the time, especially IFR. Taking one of them off the table is not on my agenda. The flashing light would be indication enough. I've never had the thing trigger in flight anyway. It's always been some sort of manhandling on the ground with the engine not running.
The installation instructions would have one running the self-test every 3 months. Not a terrible idea. There are various codes emitted by the buzzer to depict various anomalies.
